- Mon-Fri 7:30 AM to 4:30 PM
Primary Responsibilities:
- Client Communication: Serve as the primary point of contact for clients, addressing inquiries, concerns, scheduling updates, and feedback in a professional and timely manner.
- Client Onboarding: Conduct comprehensive client assessments, devising initial care plans that pave the way for a successful, personalized care experience.
- Care Plan and Care Program Development & Updates: Regularly update care plans and programs based on caregiver feedback, visit notes, identified issues, and routine three-month reviews, ensuring alignment with client needs and goals.
- Caregiver Coaching – Client Care: Provide coaching to caregivers to improve the quality of care delivered and address any issues that arise during service delivery.
- Client Satisfaction Check-Ins: Conduct regular check-ins with clients to ensure satisfaction and encourage feedback, including gathering reviews for platforms like Google and Yelp.
- Regional Meetings: Participate in regional meetings, sharing insights and fostering collaboration to ensure continuous improvement in service delivery.
- Documentation: Maintain detailed records of all interactions to ensure service continuity and effective knowledge transfer.
Qualifications:
- At least one year of experience working for a home care agency, assisted living facility or memory care facility.
- At least two years of experience in care coordination (creating care plans, performing client assessments, etc.)
- At lease one year of experience working in a remote environment.
- At least one year of experience overseeing a team of caregivers.
- A Bachelor’s degree in a related field is preferred but not required.
- Is an excellent communicator, detail-oriented, and empathetic.
